Services
Comprehensive Evaluation
Your initial evaluation will consist of a comprehensive evaluation of your current symptoms, medical history, past treatments, family history, lifestyle, dietary habits, environmental factors, risk factors, goals, and individual views of different therapeutic modalities.
Lab Evaluation
Ordering and interpretation of comprehensive labs based on evaluation and ongoing needs. May include genetic testing, hormone evaluation, assessment for nutritional or vitamin deficiencies and imbalances, gastrointestinal health, inflammatory markers, or environmental stressors.
Medication Management
Medication management includes assessment of the need for psychopharmacology, prescription management, medication education, review of your complete medication list, and deprescribing when appropriate and through shared decision making.
Therapy
Offering individual integrative therapy sessions drawing from solution-focused therapy, motivational interviewing, mindfulness techniques, and psychoeducation.
Complementary and Holistic Therapies
Education and personalized recommendations for complementary and holistic therapies. May include nutrition planning, supplement recommendations, mind-body therapies, and nature prescriptions.
Prevention Planning
Bringing it all together to help you find your daily floss.

All services are offered via telehealth to anyone residing in the state of Oregon. In-person visits are available by appointment in Roseburg, OR
Rates

This is your initial 90 minute evaluation which includes initial treatment plan and recommendations. Please contact for cost of session.

Includes medication management and/or therapy sessions.
30-60 minute sessions. Please contact for costs of sessions.

Routine labs: may be covered by insurance, when not covered prices vary greatly.
Specialized Functional Lab Testing: range $ 200 - $ 600, insurance may cover partially.

In-network: Regence BlueCross BlueShield of Oregon and Pacific Source
Out-of-network: A superbill can be provided for your visit if you have a PPO plan that allows for out of network reimbursement. For more information, it is recommended to ask your insurance about out of network benefits and reimbursement.

In compliance with the “No Surprises Act” starting on 1/1/2022 “Good Faith Estimates” are available upon request and will be provided to all patients within 3 business days.
